The Hopsital Discharge We assist the NHS in the Discharge of Patients and we aim to provide structure and a stress free experience back to the community.  

We also provide information to the service user on the availability of all known services available to them and their purpose which may include voluntary, support services, government and other regulated providers as well as a meaningful, listening, caring and conversational service. We understand the service user and their relatives sometimes have no idea what the long term effect of their diagnosis is and who and what service is there to support them. We hope we can help with information so you can get in touch with different organisations that are available. 
 
The care is met by Health Care Assistants supported by the District Nursing Service as well as Telopea MSL Community Nurses who are working and liaising together for the best outcome and care of the patient. With Telopea MSL there is one care plan for all the carers managed and in place, whether it is necessary to cover that particular care with 3-4 agencies, just 1 agency or Telopea MSL only, all will be managed and all will write in the same care plan for continuity of care. A communication book may also be available and is very useful for all visiting services. 
The District Nurse Team providing any Nursing remit will maintain their own documentation in the home and reference any particular request in the communication book for liaison of other services. 
 
For the service user each team remains invaluable because all services are important, with different professions coming together to provide the best quality of life to the Service User, keeping everyone informed. 
 
Membership of domiciliary agencies to Telopea MSL means that we can put the right ‘fit’ of care to the right level of service on discharge because it is assessed and then known. Telopea MSL will gain as much professional information as can be expected to identify the immediate needs of the person and subsequently modify the care package with agreement as the care settles and changes. 
 
Telopea MSL also expects to be efficient in liaising with Social Services team, District Nurse team and the relevant GP to assist and inform the persons changes and requirements. 
 
We trust that by using the services of Telopea MSL the quality of care will be enhanced and less fragmented and we will make future inspections to ensure first class care continues to be in place.
